# Semaglutide and the events a trial can prevent

> Semaglutide outcomes | Averta Peptides — Averta Peptides on semaglutide: cardiovascular and kidney outcomes, weight loss in patient trials, digestive effects and unresolved rare risks.

Weight change is conspicuous. Cardiovascular and kidney outcomes give semaglutide's larger patient trials another, quieter subject.

## SELECT

SELECT's combined outcome covered cardiovascular death, nonfatal heart attack and nonfatal stroke. [SELECT enrolled 17,604 adults](https://pubmed.ncbi.nlm.nih.gov/37952131/) with established cardiovascular disease and overweight or obesity, without diabetes. The hazard ratio for that combined outcome was 0.80 with semaglutide compared with placebo.

A hazard ratio compares how quickly events occur during follow-up. It describes the relative difference between groups; the number of events avoided also depends on the starting risk and the time observed. The trial's population therefore deserves as much attention as its headline figure.

I prize the cardiovascular finding for its clinical consequence. A vague claim about metabolic optimisation offers far less to judge. Still, SELECT concerns people with existing cardiovascular disease. Extending that result to everyone interested in weight loss would give it a reach the trial never tested.

## FLOW

[FLOW studied 3,533 adults with type 2 diabetes and chronic kidney disease](https://pubmed.ncbi.nlm.nih.gov/38785209/). Its combined kidney outcome included kidney failure, a major decline in kidney filtration, or death from kidney or cardiovascular causes. The hazard ratio was 0.76 against placebo. That combined endpoint makes the clinical stakes much clearer than a general claim of organ support.

The kidney and cardiovascular trials involved different populations and different outcomes. Ranking their hazard ratios as though they measured the same protection would obscure both results. The concrete problems they counted — worsening kidney function and serious cardiovascular events — give each trial its own clinical stakes.

## STEP 1; SURMOUNT-5

In [STEP 1, adults with overweight or obesity without diabetes](https://pubmed.ncbi.nlm.nih.gov/33567185/) had mean body-weight changes of −14.9% with semaglutide and −2.4% with placebo at 68 weeks. Those are group averages from that trial. They give no personal timetable for weight loss.

[SURMOUNT-5 compared tirzepatide directly with semaglutide](https://pubmed.ncbi.nlm.nih.gov/40353578/) in adults with obesity. Mean weight changes at 72 weeks were −20.2% and −13.7%, respectively. That comparison answers a weight question. The cardiovascular result above answers a different clinical question, so the larger weight change cannot settle which treatment prevents more cardiovascular events.

[Semaglutide mimics GLP-1](https://pubmed.ncbi.nlm.nih.gov/34305810/), a gut hormone involved in appetite and blood-sugar regulation. It strengthens insulin release when glucose is high, suppresses inappropriate glucagon release and slows stomach emptying. Its effects on hunger and fullness help explain reduced food intake. An explanation of appetite remains distinct from a measured cardiovascular or kidney outcome.

## Semaglutide safety

The [semaglutide safety review](https://pubmed.ncbi.nlm.nih.gov/34305810/) judged the overall benefit–risk profile favourable in type 2 diabetes. Nausea, vomiting, diarrhea and constipation dominated the adverse effects; some participants discontinued treatment. Gallbladder disease, including gallstones, occurred more often.

Rodent thyroid tumours underlie the thyroid warning. The available human evidence has not established a clear increase in thyroid cancer, and rare pancreatic and thyroid cancer questions remain difficult to resolve. Rapid blood-sugar improvement can also worsen existing diabetic eye disease. These concerns need their own attention rather than being absorbed into an attractive average weight change.

Semaglutide has approved prescription uses for particular conditions and formulations. Research material has separate uncertainties about identity, strength and purity; a trial of the studied medicine cannot verify another preparation.

## Appetite accounts

Community descriptions often dwell on thinking less about food, reduced cravings and earlier fullness. Other accounts describe nausea, sulfur-smelling burps, constipation, diarrhea, reflux, fatigue or unpleasant changes in taste. These are anecdotal accounts, not clinical evidence; they cannot establish cause or the frequency of a problem.

The ordinary texture of those descriptions interests me. A trial can count a serious event, while daily tolerability may determine whether someone continues treatment at all. Both questions merit attention, with the clinical trials supplying the measured estimates. The [comparison](/compare) places these findings beside the much smaller human record for BPC-157 and the preclinical work on KPV.
